Report Internet Choice Denial

This form's intended use is for the reporting and documenting of the denial of Internet Choice to occupants of multi-unit buildings of 4 or more units. The information you submit here will help the City inform landlords of the occupant's rights to choose the Internet provider of their choice and enforce Section 8.66 of the Municipal Code, the Communications Service Provider Ordinance (2021).
